My Voice, My Choice is a grade card for providers, that is created by individuals with disabilities and their families.  The purpose of the project is to interview individuals that receive Residential Services and determine their level of satisfaction with their Service Provider.  Individuals were asked to complete a 15-question “report card” on their Residential Provider.
